When you arrive, we will need to
make a copy of a government issued photo id and all insurance cards
that you may have.
We would encourage you to print the new patient paperwork forms
from our website, fill them out prior to your visit and carry those
in at the time you see the doctor. You also have the option of
filling the paperwork out once you are in the office. It is important
to bring a list of all current medications that you may be taking.
You will need to be prepared to pay any co-pay that your insurance
will require. If you have HMO or Medicaid insurance plans, you
will need to have a referral from your primary care physician.
